Targeted Service Coordination

A Targeted Service Coordinator is an individual who is an unaffiliated, unbiased advocate and liaison between BDDS, your service providers, and your support team. A service coordinator assists in developing and carrying out a plan for Medicaid recipients that receive DD waiver, state plan and A&D waiver services.

Plan Development

Targeted Service Coordinators have specific training in Person Centered Planning and are the entity that assist you in becoming eligible for Medicaid waivered services through the Bureau of Developmental Disabilities. Once you become eligible, they assist in developing a plan based off an individual’s unique needs that guide their service structure and outline how they will spend their budget dollars. This process is called Plan Development. Post Plan Development, the TSC provides continual support and oversight to the individual and their Medicaid plan in a case management type structure/role.

Transition Management

Transition management services are available to individuals who have been living in a qualified Medicaid funded institutional setting for a minimum of 45 days. We provide relocation assistance and intensive service coordination activities to assist individuals transition into the community setting of choice.

Appeal Training

All TSC’s are appeal certified and trained. This applies to initial or existing DD Waiver and State Plan services. This occurs when there is not enough budget dollars to utilize Medicaid services to address health and safety needs/concerns.

Behavior Consulting (COMING SOON!)

Behavior Consulting is a new service array that COMMUNITY NOW and H&W are working towards implementing in June of 2022. If this service is to become part of the selection, it will focus on analyzing individuals who are struggling with behavioral problems in various environments and apply focused interventions to assist them in making behavioral changes to optimize their own health and safety.

Counseling

Counseling helps individuals, families, and groups feel empowered to accomplish mental health, wellness, career, and life goals. It involves making needed changes in our ways of thinking, feeling, and behaving. It is a collaborative process, one that is free from judgment, one that is met with unconditional positive regard, and one that is safe, trustworthy.

Case Management (COMING SOON!)

Mental Health Case Management is the coordination of community-based services by a professional or a team to provide people the quality mental health care that is customized to an individual’s treatment needs. This service can be accessed along with counseling services and is ideal for those who need additional help in stabilizing and preventing further, future crises from developing.
